29 CFR 1910.147(d): The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions were not done in the following sequence. a. Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about May 25, 2017, the employer did not ensure an employee performing puck change-outs on the OD Grinder located in Cell #1 locked out the electrical, pneumatic, and gravitational energy prior to performing the work, exposing the employee to struck by and caught-in hazards. b. Ross Casting & Innovation, LLC located in Sidney, Ohio: On or about May 25, 2017, the employer did not ensure an employee performing saw puck change-outs on the Auto Saw located in Cell #1 locked out the electrical and pneumatic energy prior to performing the work exposing the employee to struck-by and severe laceration hazards. The machines were not shut down or turned off prior to engaging in these servicing and maintenance activity. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4]), dissipation of residual energy [1910.147(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine servicing hazards.
