29 CFR 1926.800(k)(1)(ii): Mechanical ventilation was not provided in all underground construction work: a) Culvert Install Site: On or about May 05, 2017 and at times prior, as the controlling employer, Knife River did not ensure that its sub-contractor, Coleman Construction Inc., protected their employees in that employees were exposed to acute illness, burns, impairment of ability to self-rescue, and incapacitation while removing soil, inspecting, flame cutting, tacking and welding inside of a 36 inch culvert. The controlling employer failed to ensure that Coleman Construction Inc., provided mechanical ventilation to prevent dangerous or harmful accumulations of dusts, fumes, mists, vapors, or gases.
