29 CFR 1910.333(b)(2)(iv)(B): Before circuits or equipment was worked as de-energized, a qualified person did not use test equipment to test the circuit elements and electrical parts of equipment to which employees would be exposed and did not verify that the circuit elements and electrical parts were de-energized: Wave Wash Room: On, or about, February 24, 2017, the employer did not perform testing to ensure that a drop cord that was selected to energize a new receptacle was de-energized prior to an employee attempting working on the circuit. The employee suffered serious electrical burns and shock.
