29 CFR 1910.146(g)(1): The employer did not provide training so that all employees whose work was regulated by 29 CFR 1910.146 (permit required confined spaces) acquired the understanding, knowledge, and skills necessary for the safe performance of the duties assigned under 29 CFR 1910.146: The employer is failing to protect employees from the hazards associated with permit required confined space entries. This was most recently documented on February 1, 2017 at 9100 West Liberty Drive, Pleasant Valley, Missouri 64068. The employer is not training employees to recognize that breaking the plane constitutes entry into a permit required confined space.
