On or about August 8, 2016, and at times prior to and thereafter, the employer did not perform inspections and tests of the wet out process equipment, including, but not limited to, the following: a) The employer did not inspect and test the temperature indicator (thermocouple) on the small Mix Tank. b) The employer did not inspect and test the temperature indicators (thermocouples) on the resin tanks. c) The employer did not inspect and test the automated glycol heating/cooling system for the resin tanks.
