Paragon Tempered Glass, LLC - Antwerp, Ohio: On or about December 22, 2016, the employer did not ensure that the bloodborne training program included elements as required by 29 CFR 1910.1030(g)(vii)[A] through [N]. The training program did not include: accessing the regulatory text, an explanation and copy of a written plan, information on the hepatitis B vaccination, procedures to take and who to contact in an emergency involving blood, procedures in the event of exposure, and information on post-exposure evaluation.
