29 CFR 1910.119(j)(4)(ii): Inspection and testing procedures did not follow recognized and generally accepted good engineering practices (RAGAGEP). On or about June 21, 2016 and at times prior thereto, the employer did not follow recognized and generally accepted good engineering practices (RAGAGEP) for inspection of the mechanical ventilation system in the engine room at American Cold Storage located in Tulsa, Oklahoma. Examples of such RAGAGEP include but are not limited to IIAR Bulletin #111 (Ammonia Machine Room Ventilation) and/or IIAR-2 (Equipment, Design, and Installation of Ammonia Mechanical Refrigerating Systems). The employer failed to perform required testing for Ventilation Fans and Intake: a. Stop the non-emergency continuous ventilation fan(s) using the appropriate electrical disconnect(s). Confirm that the alarm for non-emergency continuous ventilation functions properly. b. Using ampules or cylinders of pre-mixed anhydrous ammonia test gases; expose each of the ammonia detectors in the machinery room to ammonia concentrations above the alarm level. Confirm that individual detector(s), control circuitry, ventilation fans, dampers, and alarms function properly.
