29 CFR 1910.333(a): Safety-related work practices were not employed to prevent electric shock or other injuries resulting from either direct or indirect electrical contacts, when work was performed near or on equipment or circuits which were or could be energized: a.) Terminal Ready Mix, Inc. located at 524 Colorado Ave. Lorain, OH; On or about May 8, 2016, the employer did not ensure employees utilized electrical safety related work practices while pulling 480V fuses on the Plant Cement Mixer. Employees were required to perform work on the Plant Cement Mixer and pulled the 480V fuses to ensure energy control and were exposed to electric shock and arc flash hazards.
