29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer: (a) On or about July 7, 2016, the employer failed to provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer. Employees working in the spray area were exposed to respiratory hazards including but not limited to Elgen Water Based Liner Adhesive, Capital Hardware Elgen 100% Silicone Sealant and Adhesive, Elgen Duct Sealer, and OEM/Mechanical - CT10101-5 insulation material while voluntarily using N-95 and half-face elastomeric respirators. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
