29 CFR 1910.27(b)(1)(ii): The distance between rungs, cleats, and steps shall not exceed 12 inches and shall be uniform throughout the length of the ladder. On or about May 12, 2016, the employer failed to ensure the distance between the rungs on the fixed ladder, located in Building 155, in the Hydro Department at Column G:13, was approximately 15 inches, exposing employees to fall hazards.
