29 CFR 1926.452(i)(4): Outrigger beams were not secured in place against movement, and were not securely braced at the fulcrum point against tipping. On or about August 25, 2015 a) 2nd floor Southwest elevator shaft: The elevator shaft scaffold was erected using 2 Aluminum outrigger I-beams with wood planks and plywood. One outrigger beam was not secured using a shoring post. An employee fell from the platform when the platform partially collapsed.
