29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: A.) Warren Railcar Service Inc. - On or about 8/19/15, and at times prior thereto, employees were performing servicing and maintenance work on machines and equipment that include, but were not limited to, cranes, press break, and front end loader. The authorized employee performing the work did not affix their own lockout device. The lockout device was applied/controlled by the supervisor.
