29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect the safe operation or mechanical strength of the equipment, such as parts that were broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating: a) K Plant, electrical panels behind the hydraulic presses, 6th and Center Streets, Tarentum, Pennsylvania 15084: On or about July 28, 2015, the #6 press motor control center had bent doors and broken disconnects. The #6 press motor control center housed energized conductors (480 volt, 3 phase) and was labeled as a Level 0 arc flash and electric shock hazard. Employees walking or operating forklifts next to the #6 press motor control center were exposed to electric shock hazards. b) K Plant, electrical panels behind the hydraulic presses, 6th and Center Streets, Tarentum, Pennsylvania 15084: On or about July 28, 2015, there were broken or damaged parts on the #1 press motor control center cabinet. The panel labeled "2C" would not stay shut due to a missing bolt. The #1 press motor control center housed energized conductors (480 volt, 3 phase) and was labeled as a Level 4 arc flash and electric shock hazard. Employees walking or operating forklifts next to the #6 press motor control center were exposed to electric shock hazards.
