29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: On or about May 20, 2015, employees removing incorrectly packaged/jammed meat/meat packages/packaging material from the cut/seal section of the Ameripak packaging machine had not been trained to use lockout/tagout when placing their hands inside of the inadequately guarded machine. The employer has removed this machine from service. No abatement certification or documentation is required for this item.
