29 CFR 1926.452(i)(1): The employer did not ensure that the inboard end of outrigger beams, measured from the fulcrum point to the extreme point of anchorage, were not less than one and one-half times the outboard end in length. On or about August 25, 2015 a) 2nd floor Southwest elevator shaft: The elevator shaft scaffold was erected using two Aluminum outrigger I-beams with wood planks and plywood. The outrigger beams did not meet the standard's requirements. b) Southwest 7: The 2 freight elevator shaft scaffolds were erected using three Aluminum outrigger I-beams with wood planks and plywood. The outrigger beams did not meet the standard's requirements.
