29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) In Shop 3, On Aug. 4, 2014 and at times prior thereto; the employer manufactured fork extensions 102 inches long for use on all of the forklifts. b) In Shop 3, On Aug. 4, 2014 and at times prior thereto; the employer manufactured steel man-baskets for use on all the forklifts. c) In Shop 3, On Aug. 4, 2014 and at times prior thereto; the employer manufactured a "cherry picker" or jib with a hand crank winch for use on all the forklifts. Note: Abatement certification AND documentation are required for this item.
