29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a. Facility yard - On or about January 15, 2014, the written energy control program did not include specific step by step procedures to control and lockout all energy sources for employees who performed service and or maintenance on such grinding equipment including, but not limited to, a pre-screener, conveyor belts and stationary mill. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
