29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or other means: On or about May 30, 2013, employees were exposed to an electrical hazard, while plugging an extension cord into 120VAC electrical receptacle which had approximately 8 inches of exposed live wiring between the conduit and the electrical receptacle box.
