29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) On or about March 6, 2013 and at times prior, the opening between the abrasive wheel and work rest of a Stony Craft 6 inch bench grinder (Model HG-6A) was measured at 1 1/2 inch. Employees used the abrasive wheel for grinding metals and were exposed to serious injuries.
