29 CFR 1926.1437(n)(5): The physical attachment, corralling, rail system, or centerline cable system did not meet the requirements of options 1 through 4 of this section, or meet the requirements of paragraph (n)(5)(v) of this section. On and about July 5, 2012, the employer did not ensure that either the Terex HC110 or American 9299 crawler cranes placed on barges were properly secured to prevent rolling, sliding, or shifting away from its proper location. The crane was secured to the track beams set on timbers and not secured to the deck of the barge. Employees were thereby exposed to the hazards associated with crane and load failures.
