The employer did not establish a process evaluation to address industry recognized hazards, previous incidents, engineering and administrative controls, consequences for failure to documenting administrative controls and engineering controls, facility siting, human factors and evaluation of safety and health of the employees workplace. Employees were exposed to fire/explosion hazards, chemical run away reactions, and other hazards while performing assigned duties for the manufacture of industrial chemicals.
