The employer does not implement a written permit space entry program that complied with 29 CFR 1910.146: 29 CFR 1910.146(c)(4): When the employer decided that its employees would enter permit spaces, the employer did not implement a written permit space entry program that complied with 29 CFR 1910.146: A recent example of this occurred on 10/25/11 and prior, when: a) In the production/tank alley areas, Employees entered inside of the Permit-Required Confined Space: Cream Tank #3 without utilizing Lockout/Tagout Procedures to isolate tank's agitator. To abate this violation, the employer shall implement a written permit-required confined space program that includes isolating the permit space as per 29 CFR 1910.146. Pursuant to 29 CFR 1903.19(d), the employer must submit documents describing the steps it is taking to ensure compliance, within 30 days of the date it receives this citation. ABATEMENT DOCUMENTATION REQUIRED
