Text For Citation: 01 Item/Group: 005A Hazard: CRUSHING
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to a fall, struck-by, and crushing hazards while using a platform supported by a rough terrain forklift. (a)The wooden pallet being used as a work platform was approximate 12 feet in length and exceeded the allowable width capacity of the rough terrain forklift in that the width of the platform exceeded the maximum ten inches allowed beyond the load bearing tires. This exposed the employee to a tipping/fall hazard of approximately 22 feet. Among other methods, feasible and acceptable means to correct this hazard is to: (a)Ensure that the platform and rough terrain forklift are compatible for wheel width to platform width in accordance with American National Standards Institute ANSI/ITSDF B56.6-2005, Safety Standard for Rough Terrain Forklift Trucks, Section 8.24.1(b). This section states: floor dimensions shall not exceed two times the load center distance listed on the rough terrain forklift truck nameplate, measured parallel to the longitudinal center plane of the truck nor have a width greater than the overall width of the truck [measured across the load bearing tires plus 10 inches (250mm) on either side]. (b)Ensure that any platform used is a JLG approved work platform as stated in the Skytrak Operations and Safety Manual on page 1-8. Certification, and documentation/evidence of abatement is required. Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within ten (10) calendar days after the abatement date.ment
