29 CFR 1910.119(j)(3): The employer did not train each employee involved in maintaining the on-going integrity of process equipment in the procedures applicable to the employee's job tasks to ensure that the employee can perform the job tasks in a safe manner: a) In the 13, 14 Battery Transfer Area Pad Valve Pit, on or about August 11, 2025 - The employer did not train each employee involved in maintaining the on-going integrity of 13 Pad Valve in the procedures applicable to wash the valve with water to ensure that the employee can perform the wash in a safe manner. This exposed employees washing the valve, as well as other employees working on or adjacent to the 13, 14 Battery Transfer Area to an explosion when 13 Pad Valve ruptured during washing and released cryogenically-processed coke oven gas from the Underfire lines.
