Text For Citation: 01 Item/Group: 005A Hazard:
29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that had been locked out or tagged out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished. On or about 25 June 2025, Gilster-Mary Lee Corporation, located at 615 Old St. Marys Road in Perryville, Missouri, failed to verify equipment was isolated and de-energized prior to the start of maintenance activities. The employer failed to verify equipment de-energization after power to the operating panel was turned off. The lack of verification exposed employees to amputation and crushing hazards while they worked inside and around the Sugar Coat 1 Oven.
