29 CFR 1910.184(f)(5)(iii): Wire rope sling(s) that had kinking, crushing, bird caging or other damage resulting in distortion of the wire rope structure were not immediately removed from service: Vehtek Systems, Inc. - Bowling Green, Ohio: On or about March 10, 2025, the employer did not ensure that damaged and defective wire-rope slings were removed from service. Employees were exposed to hazards associated with falling loads when using kinked and distorted wire-rope slings to lift a 5,150 lb. RH Check Fixture. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
