29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Hammond's Candies, 5735 N Washington Street, Denver CO 80216: On or about January 24, 2025, employees were allowed to operate powered industrial trucks (Crown Fork Lifts 5200 Series (Serial #9A186785)) at the site without having received formal instruction, including operator evaluation to determine the retention of the information. The untrained operator(s) and others working in the immediate area were potentially exposed to crushing and struck-by hazards.
