29 CFR 1910.101(b):"Compressed gases." The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tank cars, or motor vehicle cargo tanks shall be in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in Sec. 1910.6. On or about August 14th, 2024, employees engaged the maintenance of equipment as part of the bakery production activities located at 4657 G Street, Omaha, Nebraska 68117, were exposed to fire, explosion and struck-by hazards in that full or partially full oxygen and acetylene cylinders were observed free standing and unsecured. Instances of failure include but are not limited to: a) One acetylene cylinder was free standing in the maintenance shop directly in front of a flammable materials cabinet. b) One oxygen cylinder was free standing in the boiler room. c) At least 16 oxygen cylinders were not properly secured outside the building behind the boiler room - The chain in use was not securing cylinders from tip-over. d) Two oxygen cylinders were free standing outside the main entrance of the maintenance shop.
