29 CFR 1910.1000(a)(2): Employee(s) were exposed to an airborne concentration of Nickel, Cobalt and Total Dust listed in Table Z-1 in excess of the 8-hour Time Weighted Average concentration of 1 mg/m3 for Nickel, 0.1 mg/m3 for Cobalt and 15 mg/m3 for Total Dust: a) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Nickel in that, the employer did not ensure that employees dumping bags of powder were not exposed to Nickel in excess of the permissible exposure limit (PEL) at 1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 24.46 mg/m3, during 448 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 32 minutes not sampled. b) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Nickel in that, the employer did not ensure that employees involved with cleaning and loading bags of powder were not exposed to Nickel in excess of the permissible exposure limit (PEL) at 1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 3.73 mg/m3, during 404 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 76 minutes not sampled. c) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Nickel in that, the employer did not ensure that employees dumping bags of powder were not exposed to Nickel in excess of the permissible exposure limit (PEL) at 1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 11.55 mg/m3, during 446 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 34 minutes not sampled. d) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Nickel in that, the employer did not ensure that employees involved with cleaning and loading bags of powder were not exposed to Nickel in excess of the permissible exposure limit (PEL) at 1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 32.01 mg/m3, during 439 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 41 minutes not sampled. e) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Cobalt in that, the employer did not ensure that employees dumping bags of powder were not exposed to Cobalt in excess of the permissible exposure limit (PEL) at 0.1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 4.02 mg/m3, during 448 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 32 minutes not sampled. f) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Cobalt in that, the employer did not ensure that employees involved with cleaning and loading bags of powder were not exposed to Cobalt in excess of the permissible exposure limit (PEL) at 0.1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 0.63 mg/m3, during 404 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 76 minutes not sampled. g) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Cobalt in that, the employer did not ensure that employees dumping bags of powder were not exposed to Cobalt in excess of the permissible exposure limit (PEL) at 0.1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 1.90 mg/m3, during 446 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 34 minutes not sampled. h) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Cobalt in that, the employer did not ensure that employees involved with cleaning and loading bags of powder were not exposed to Cobalt in excess of the permissible exposure limit (PEL) at 0.1 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 5.21 mg/m3, during 439 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 41 minutes not sampled. i) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Total Dust in that, the employer did not ensure that employees dumping bags of powder were not exposed to Particulates Not Otherwise Regulated (PNOR) in excess of the permissible exposure limit (PEL) at 15 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 69.07 mg/m3, during 448 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 32 minutes not sampled. j) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Total Dust in that, the employer did not ensure that employees dumping bags of powder were not exposed to Particulates Not Otherwise Regulated (PNOR) in excess of the permissible exposure limit (PEL) at 15 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 21.37 mg/m3, during 446 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 34 minutes not sampled. k) Cathode Powder Mixing Room/ Electrode Department- On or about 6/30/2023, and at times prior, the employer exposed employees to respiratory hazards associated with Total Dust in that, the employer did not ensure that employees involved with cleaning and loading bags of powder were not exposed to Particulates Not Otherwise Regulated (PNOR) in excess of the permissible exposure limit (PEL) at 15 mg/m3 which was calculated as an 8-hour time weighted average (TWA) of 40.24 mg/m3, during 439 minute sampling performed on 6/30/2023. Zero exposure was assumed for the 41 minutes not sampled.
