29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Wheelhouse - On or about October 25, 2022, and times prior thereto, employees utilized a relocatable power tap connected in series to a second relocatable power tap on the wall facing the stern of the vessel. Power source was used to power multiple heaters, exposing employees to electrical and fire hazards. b) Wheelhouse - On or about October 25, 2022, and times prior thereto, employees utilized a relocatable power tap connected in series to a second relocatable power tap on the wall facing the bow of the vessel. Power source was used to power a printer, exposing employees to electrical and fire hazards. c) Captains Room - On or about October 25, 2022, and times prior thereto, an employee utilized a relocatable power tap for power of appliances to include microwave and fridge. The relocatable power tap had been permanently affixed to flexible cord attached to the wall powering the outlet for use, exposing employees to electrical and fire hazards. d) Room 2 - On or about October 25, 2022, and times prior thereto, the Owner and Managing member of the pacific producer had permanently affixed a relocatable connected in series to a second relocatable power tap on the wall with sink and mirror. The relocatable power taps were for use of powering devices such as lighting fixtures and phone chargers, exposing employees to fire and electrical hazards. e) Oficina - On or about October 25, 2022, and times prior thereto, the Owner and managing member of the Pacific Producer had permanently affixed and utilized multiple relocatable power taps as an additional outlet source for microwave, chargers, and computer equipment. g) Main Deck/Outside Galley - On or about October 25, 2022, and times prior thereto, an outlet receptacle was not used in accordance with listing and labeling when installed and used with series connection of extension cords and was placed on the floor when utilized as a power source for refrigerators, exposing employees to electrical and fire hazards. h) Main Deck/Outside Galley - On or about October 25, 2022, and times prior thereto, extension cords were connected together in series outside, and a second extension cord was modified at the female end and hardwired for permanent installation into an outlet receptable, exposing employees to electrical and fire hazards.
