29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) 158 West Jackson St, New Holland, PA, IMPACO packaging room, scissor lift table - On or about July 14, 2022, employees were exposed to caught in hazards caused by ingoing nip points and rotating scissor action during the operation of the two scissor lift tables which were bolted on top of each other to create two separate exposure areas within 16 inches of where the cardboard (siding) was loaded into the IMPACO box assembly machine. (b) 158 West Jackson St, New Holland, PA, IMPACO Room, Butler Splicer - On or about July 14, 2022, employees were exposed to ingoing nip points caused by rotating splice rollers which were 15 inches wide located at the back of the Butler Splicer machine. (c) 158 West Jackson St, New Holland, PA, IMPACO Room, IMPACO packaging machine - On or about July 14, 2022, the sterilization door in the IMPACO packaging machine did not provide adequate guarding for employees working at the machine due to a missing 5.5 inch plug which was not in place where the sterilization hose is fed through the sterilization door which exposed employees to mechanical points of operation and rotating parts on the IMPACO packaging machine. (d) 158 West Jackson St, New Holland, PA, Packaging, 331 Bright Stacker - On or about July 14, 2022, employees were exposed to caught in hazards while working near the suction arm intake section which was approximately 42.5 inches above the ground level and the opening was approximately 45.5 inches from the table where the cardboard is placed to the bottom of the suction arm; and also at the approximately 41 inch wide platform with 3 1-inch wide exit conveyor chain pallet movers where employees work within 1 foot of the rotating chain. (e) 158 West Jackson St, New Holland, PA, Packaging, 332 Bright Stacker - On or about July 14, 2022, employees were exposed to caught in hazards while working near ingoing nip points caused by the approximately 41 inch wide platform which was approximately 14 inches above the floor level with 3 1-inch wide exit conveyor chain pallet movers. (f) 158 West Jackson St, New Holland, PA, Packaging, IMPACO Robot - On or about July 14, 2022, employees were exposed to caught in hazards while working near ingoing nip points caused by the 3 1-inch wide exit conveyor chain pallet movers which were approximately 24 inches above the floor level with. (g) 158 West Jackson St, New Holland, PA, Packaging Maintenance shop - On or about July 14, 2022, the exposed blades of a Kysor Johnson bandsaw with two saw blades which were each within three (3) inches away from being flush with the outer edge of the equipment housing were not guarded at the unused portion of the blade. Employees were exposed to a caught in hazard and amputation hazard. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
