29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): (a) On or about 4/06/2022, 5204 Hannas Rest Frederiksted, VI 00840 - Warehouse - the Toyota LP gas (Model: 8FGCU25, Serial No: 36194, Capacity: 5000) sit-down power industrial truck was not being operated by a competent operator, exposing employees to struck-by hazard.
