29 CFR 1910.146(d)(3): Develop and implement the means, procedures, and practices necessary for safe permit space entry operations, including, but not limited to, the following: (i) Specifying acceptable entry conditions; (iii) Isolating the permit spaces. a) On or about February 25, 2022, the line operator for Line 2 Nova Hopper, a permit-required confined space, entered the space without implementing the procedures to safely enter the space such as isolating the chain drive.
