29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a. 200 First St., Elizabeth, NJ The employer did not establish a written hazardous energy control program that included employee training, equipment specific procedures, and periodic inspections. Employees engaged in the maintenance and repair of equipment that included but was not limited to a Genox Machine wire shredding machine, were not provided a written LOTO program which included training or periodic (annual) audits of procedures for the equipment that would be required to be isolated from an energy source and rendered inoperative. Violation occurred on or about February 18, 2022
