29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a) On or about December 29, 2021, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in hazards when the conveyor belt on the infeed side of the Line #1 Slitter/Rewinder was not guarded to prevent exposure to an in-running nip point. b) On or about January 10, 2022, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when the door acting as guards on the upper level of the Card Deck on Line #2 did not prevent access to the areas where multiple moving machine part, including rotating cylinders, were in operation. c) On or about January 10, 2022, with respect to an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in hazards when the in-running nip points for the apron inside the Perfo-Jet area were not guarded to prevent access. d) On or about January 19, 2022, with respect to an establishment located at 1330 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in and struck-by hazards when two interlocked doors on the upper level of the Card Deck for Line #3 had been left in a defeated state with the machine still in operation. e) On or about January 19, 2022, with respect to an establishment located at 1330 Glory Road, Green Bay, Wisconsin, employees were exposed to potential caught-in when the in-running nip points for the apron inside the Aqua Jet area were not guarded to prevent access. f) On or about December 31, 2021, at an establishment located at 1330 Glory Road, Green Bay, Wisconsin, employees were exposed to machine hazards associated with moving parts when they engaged in "stoning" activities on Card 2 of Line #3. The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to complete the stoning task. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity. g) On or about December 29, 2021, at an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to machine hazards associated with moving parts when they engaged in threading activities on the Padder/Aperture Section on Line #1. The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to thread the web into rollers. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity. h) On or about January 10, 2022, at an establishment located at 1250 Glory Road, Green Bay, Wisconsin, employees were exposed to machine hazards associated with moving parts when they engaged in threading activities on the Perfo-Jet Section on Line #2. The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to thread the web into rollers. The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.
