29 CFR 1910.147(e)(2)(i): Release from lockout or tagout. Before lockout or tagout devices are removed and energy is restored to the machine or equipment, procedures shall be followed and actions taken by the authorized employee(s) to ensure the following: The work area shall be checked to ensure that all employees have been safely positioned or removed. a) On or about May 26, 2021 when the machine was in jog mode during the setup process, the work area was not checked to ensure employees were in safe position prior to jogging the machine, resulting in an employee's hand being pulled into the sizing rolls.
