On or about May 17, 2021, and times prior thereto, the employer did not ensure lockout/tagout procedure clearly and outline the scope and purpose of the procedures for the TruLaser 3030. The energy control procedures did not included a statement in the intended used of the procedure. Employee(s) were exposed to struck-by, caught-in, and amputation hazards.
