29 CFR 1926.454(a):The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. On or about March 05, 2021, on the northwest side of the project site, employees were not trained by a qualified person in hazard recognition associated with scaffolding while utilizing a scaffolding to perform masonry work on the outside of a building at heights upwards of eight (8) feet.
