29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: K&B Solutions Warehouse, sanding department/area: On or about 02/16/2021, the employer exposed employees to electrical shock hazards, in that a 50-foot extension cord used to power lighting fixtures was missing the ground pin. Employees tasked with plugging and unplugging the extension cord at the start and end of shift.
