29 CFR 1910.146(d)(3)(vi): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not develop and implement the means, procedures, and practices to verify that conditions in a permit space were acceptable for entry throughout the duration of an authorized entry: a - On or about January 28, 2021, at the worksite Foundation Food Group, Inc., located at 2076 Memorial Park Drive, Gainesville, GA, at Plant 4, in the Line 2 and Line 4 liquid nitrogen freezers, employees were exposed to mechanical, thermal and asphyxiation hazards while entering permit required confined space areas within the liquid nitrogen freezers as the employer did not implement verification procedures to ensure acceptable conditions throughout the entry.
