29 CFR 1910.146(k)(3)(i):Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, above the entrant's head, or at another point which the employer can establish presents a profile small enough for the successful removal of the entrant. a) On or about December 1, 2020 through December 11, 2020, Kroeschell Engineering Co., Inc. did not ensure that employees utilize chest or full body harness with a retrieval line attached when they entered steam vault SV1.4 at the Captain James A. Lovell Federal Health Care Center in order to remove and replace a flexible expansion joint on a 12" high pressure steam line.
