29 CFR 1910.1200(g)(2): As required by 29 CFR 1910.1200; Appendix D of the standard Table D.1. Headings 2 (Subheading (a) through (d)) and 3 (Subheading for mixtures in addition to the information required for substances (a) through (d)). The chemical manufacturer or importer preparing the safety data sheet did not list citric acid on the Material Safety Data Sheet for PureVista and ensure that all health hazards for each ingredient were listed and identified within Sections 2 (Hazard) and 3 (Composition Information on Ingredients) of the safety data sheet. On or about November 11, 2020, an explosion occurred during the manufacturing of Pure Vista, a water-activated product used to generate chlorine dioxide, and the employer did not convey specific information on the Pure Vista safety data sheet per the company's internal policy and/or briefings of known hazards involving friction sensitivity and auto ignition properties associated with the mixing of ingredients, and did not list citric acid as an ingredient on the Pure Vista safety data sheet. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
