29 CFR 1926.1204(a):(a) Implement the measures necessary to prevent unauthorized entry; a) On or about June 29, 2020, at the above address, the employer did not implement effective measures to prohibit an employee from entering the pit of the freight elevator system (a permit-required confined space) during overhead re-cabling operations, thereby exposing an employee working in the elevator's pit to struck-by hazards from overhead materials and equipment.
