1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: On or about December 17, 2019, employees were exposed to electrical hazards and the employer did not ensure that electrical equipment was effectively closed exposing employees to electrical hazards in the following instances: a) Southside of shop, white control panel for Weldwire Positioning Equipment Model RD-5-INW-A (Serial Number D-500471JA) with a missing knockout; b) Southside of shop, Square D Disconnect Labeled 012 (480V) with a missing knockout; and c) Center of shop at north submerged arc welder, Square D Disconnect (480V).
