29 CFR 1910.217(b)(14)(i): The brake monitor was not constructed so as to automatically prevent the activation of a successive stroke if the stopping time or braking distance deteriorates to a point where the safety distance being utilized does not meet the requirements set forth in paragraph (c)(3)(iii)(e) or (c)(3)(vii)(c) of this section: Instances including, but not limited to, the following: b) Job site, 33 McGovern Road, Crescent, PA: On or about November 23, 2019, during hands in die production activities on the Warco part revolution press number 2, serial number 5346, located in building 4, the employer did not ensure that the brake monitor system was constructed in such a manner that it would prevent the press from operating if the stopping time or safety distance for the 2 hand controls deteriorated to an unsafe limit. The press had an older style control system that did not perform this task and additional components that could do so were not installed on the controls.
