29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): On or about April 18, 2019, untrained employees were allowed to operate powered industrial vehicles (e.g. the company's LP forklift and electric fork trucks) at the site without having received formal instruction, including operator evaluation to determine the retention of the information.
