29 CFR 1910.303(b)(7)(i): Unused openings in boxes, raceways, auxiliary gutters, cabinets, equipment cases, or housings were not effectively closed to afford protection substantially equivalent to the wall of the equipment: On or about March 8, 2019, employees were exposed to electrical hazards because: a) the Square D Disconnect labeled 208 located in the Maintenance Shop was missing a knockout. b) the outlet box located beneath the Square D Disconnect labeled 208 located in the Maintenance Shop was missing a knockout. c) the electrical panel marked HOL located in the Main Electrical Distribution Room contained unused openings at breakers 2, 4, 6 and 18. d) the electrical panel marked TR located in the Main Telecommunications Distribution Room contained unused openings at breakers 39 through 42.
