29 CFR 1910.272(g)(1)(i): The employer did not issue a permit for entering bins, silos, or tanks and the employer or the employer's representative (who would otherwise authorize the permit) was not present during the entire operation: (a) Rice Storage Bin #14 - On or about February 14, 2019 the employer did not issue a bin entry permit and did not ensure a representative was present during the entire operation while employees were performing work inside the storage bin, exposing employees to caught-in and caught between hazards.
