29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about 12/17/2018, at 2000 Park Avenue, Miami, Florida employees were exposed to a 10 foot fall hazard while using the base section of an extension ladder when the side rails did not extend at least 3ft above the upper landing. PLEASE NOTE: DOCUMENTATION AND CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
