On or about November 28, 2018, at the elevator, the employer did not inform employees, by posting danger signs or by any other equally effective means, of the existence, location of, and the danger posed by permit spaces. This exposes employees to potentially hazardous atmospheres, falls, slips, trips, caught-by hazards, struck-by hazards, and air borne dusts that obscure vision: 1. Boot pit, and 2. Head pulley area.
